Responsible to Senior Care Co-ordinator /Nominated Individual/Registered Manager

Job purpose

The aim of the Care Co-ordinator is to co-ordinate the delivery of a safe, well planned homecare service based within the Harrogate and Knaresborough locality ensuring continuity of care planning and work allocation to peoples visits and carer allocation.

You will be responsible for staff rosters using our Birdie scheduling software system, and the onboarding of new customers and new carers of our service, ensuring the care planning system runs effectively and efficiently.

The Care Co-ordinator role will include care delivery if needed and you will be part of our on call emergency out of hours duty team.

Duties and responsibilities

  • Ensure correct input of data of service provision and delivery inputted into the rostering systems.
  • Ensure all data is kept up to date and regularly reviewed.
  • Answering and taking new referrals or changes in planning of visits over the telephone and entering these immediately onto the relevant IT and communication systems.
  • To lead by example, be professional, helpful and informative to prospective customers, relatives, Health Professionals and staff members.
  • To demonstrate effective interpersonal and business liaison skills.
  • Liaising with managers, senior support workers and carers to ensure they have accurate and detailed information to deliver a quality service.
  • Be part of our emergency on call to Carers/ Service Users on a rotational basis.
  • Work in the community as required.
  • Maintain confidentially in line with the Data Protection Act and GDPR and ensure safe storage of information/data.
  • Responding to our referral inbox and inputting new customers and carers on to our systems.
  • Ensuring our systems are kept up to date and the inputting of staff holidays and sickness.
  • Ensuring all visits are covered and allocated in the event of staff sickness or unexpected situations /events.
  • Report concerns or issues directly to Management and escalate.
  • Working to include weekend cover of holidays of the other coordinators.
  • Hold responsibility for rota planning and liaising with carers prior to their care calls and managing client visits.
  • Support new starters to Continued Care, by managing their rota and listening to their feedback as part of the Induction process

Personnel

  • Attendance in staff team meetings.
  • Correct reporting of any incidents/accidents in accordance with Health & Safety/COSHH requirements to management and escalating concerns.
  • To comply with all Continued Care policies and procedures including working time directive.
  • Keep up to date with system changes on Birdie (Care management system)
  • Awareness of Care Quality Commission regulations.
  • Awareness of safeguarding concerns recording and reporting processes.

Essential requirements of the Care Co-ordinator

  • You must have computer skills and knowledge of Word Processing, Spreadsheets and Databases.
  • Have a pleasant and confident telephone manner with all enquirers such as Social Workers, People we support, and their families.
  • Have the initiative to work by themselves but also be an efficient team member.
  • Be able to work to deadlines and under pressure.
  • Must be reliable, flexible, and committed to Continued Care.
  • Inclusion in our on call out of hours service provision (additional paid enhancement).
  • Provide flexible cover for colleagues in the event of holiday and sickness including weekend, Bank Holidays.
  • Ensure all Service Users’ and staff information is always kept confidential.
  • Demonstrate effective leadership and managerial skills
